Abstract
Indonesia's health development is directed towards achieving healthy health problem solving for the population in order to realize optimal health status, from the data, concluded that health problems will be affected by lifestyle, diet, work environment, exercise, and stress. Lifestyle changes, especially in big cities, lead to increased prevalence of degenerative diseases, one of them hypertension. The purpose of this research is to know the description of the implementation of Risk Based Hypertension Risk Factors Information System in Sidoarjo District Health Office. The design of this activity using descriptive design. The location of this research took place at the Health Office of Sidoarjo Regency. The data was collected by direct interviews by distributing questionnaires to several employees of Sidoarjo District Health Office. Direct interviews were conducted to find out the problems of what happened in the implementation of Information Systems of Posbindu-Based Hypertension Risk Factors. Non-Communicable Disease Surveillance and its risk factors are one of the strategies for prevention and control of the disease that is done precisely and integrated by a government, private and public. Constraints currently faced is the absence of accurate data related to the epidemiology of Non Communicable Diseases, data that has been collected from surveillance activities manually still have to be processed first to be presented, where in the process of management is not standardized so that there is disparity reporting format that impact to the difficulty to do aggregate data. Non-communicable diseases occur due to various risk factors, such as smoking, unhealthy diet, lack of physical activity, and consumption of alcoholic beverages. Risk factors will cause physiological changes in the human body, thus becoming risk factors such as increased blood pressure, blood sugar increased, blood cholesterol increased, and obesity..
